ABSTRACT

BACKGROUND: Total flavonoids of Rhizoma Drynariae can be used to prevent and treat osteoporosis, but its mechanism of action is mostly focused on osteoblasts rather than osteoclast differentiation. OBJECTIVE: To investigate the effect of Rhizoma Drynariae on osteoclast differentiation through T-cell nuclear factor 1 (NFATc1) signaling pathway. METHODS: Forty Sprague-Dawley rats were randomly divided into four groups. Total flavonoids of Rhizoma Drynariae were intragastrically administered at 0, 11.6, 34.8 and 104.4 g/kg, respectively. Control serum and low-, middle-, high-concentration drug-containing sera were obtained. Bone marrow macrophages were isolated from 5-week-old rats and the effects of different drug-containing sera on macrophage activity were detected by cell counting kit-8 method. The macrophages were divided into low-, middle-and high-concentration groups, negative control group and blank group, with 5 multiple holes in each well. The low, middle and high concentration of drug-containing serum, negative control serum culture medium and normal culture medium were added respectively. All culture media were used to induce osteoclast differentiation with 20 μg/L macrophage colony-stimulating factor and 100 μg/L receptor activator of nuclear factor kappa B ligand. The number of osteoclasts and fusion index were detected by tartrate-resistant acid phosphatase staining 7 days after induction of differentiation. The expressions of c-Fos, Fra-1, Fra-2, NFATc1 and cathepsin K were detected by real-time fluorescence quantitative PCR and western blot at 7 days after differentiation. The number of bone resorption lacunae and the proportion of lacunae area in osteoclasts after 14 days of differentiation were detected by bone fragment absorption lacunae test. The study protocol was approved by the Animal Experiment Ethics Committee of Anyang District Hospital of Puyang City with approval No. PYSAYDQ-2019096. RESULTS AND CONCLUSION: Cell counting kit-8 results showed that there was no significant change in macrophage activity after intervention with different concentration of sera containing Rhizoma Drynariae. The morphology of macrophages was regular before differentiation. After differentiation, osteoclasts were identified by tartrate-resistant acid phosphatase staining. Compared with the blank group and negative control group, the number of osteoclasts, fusion index, number of bone resorption lacunae and proportion of lacunae area and the relative expressions of c-Fos, Fra-1, Fra-2, NFATc1, cathepsin K mRNA and protein in the low-, middle-and high-concentration groups decreased significantly (P middle-concentration group > high-concentration group, and there were significant differences between different concentration groups (P < 0.05). In conclusion, Rhizoma Drynariae may inhibit the differentiation of rat macrophages into osteoclasts through NFATc1 signaling pathway, and the degree of differentiation inhibition is related to the serum concentration of Rhizoma Drynariae.

ABSTRACT

Objective To investigate maternal awareness and knowledge of obstructive sleep apnea syndrome (OSAS) and their compliance with polysomnography monitoring in snoring gravidas.Methods This study enrolled 589 volunteered gravidas who were treated at the 908th Hospital of Chinese People's Liberation Army Joint Logistic Support Force or Maternal and Child Health Hospital of Jiangxi Province from April 2016 to April 2017.The maternal knowledge of OSAS (0 point:complete lack of knowledge of OSAS;1 to 4 points:with partial knowledge of OSAS;5 points:correct understanding of OSAS) and the way of obtaining this knowledge were studied using a self-designed questionnaire.Influencing factors,including gestational weeks,educational background,snoring and high-risk pregnancy,were also analyzed.Chi-square test was used for statistical analysis.Results The 589 gravidas had few knowledge of OSAS and only 11 of them [1.9% (11/589)] were able to fully understand OSAS (5 points).The proportion of women who were completely lack the knowledge of OSAS (0 point) in those less-educated women was higher than in those well-educated group [61.9% (78/126) vs 52.0% (241/463),x2=3.873,P=0.049].Among the gravidas who were unaware of their snoring condition,those completely lack the knowledge of OSAS (0 point) accounted for 67.0% (209/312),which was higher than the percentage among gravidas knowing they had or did not have snoring problem [35.6% (21/59),40.8% (89/218);x2=20.755,35.687;both P<0.017].There was no significant difference in OSAS awareness among gravidas regardless of their gestations and whether they were classified as high-risk or not (all P>0.05).Of 59 gravidas with snoring,only 15 (25.4%) accepted polysomnography monitoring.Eight out of the 589 gravidas (1.4%) were diagnosed with OSAS during pregnancy.Conclusions Gravidas have poor knowledge and awareness of OSAS,especially those with low educational background and not knowing their snoring condition,resulting in poor compliance with polysornnography monitoring and low diagnostic rate of OSAS during pregnancy.

ABSTRACT

We reported a case of severe preeclampsia presented with chest tightness, dyspnea and pink foam sputum after cesarean section. The patient was diagnosed with obstructive sleep apnea hypopnea syndrome (OSAHS) with acute left heart failure based on her physical examination, medical history and polysomnogram. Symptoms of heart failure were relieved after continuous positive airway pressure ventilation.

ABSTRACT

Objective: To explore the correlation of monocyte to HDL-C ratio (MHR) and post-operative slow lfow or no relfow in patients with ST-segment elevation myocardial infarction (STEMI) after percutaneous coronary intervention (PCI). Methods: A total of 216 STEMI patients treated in our hospital from 2014-10 to 2016-05 were enrolled. The patients were divided into 2 groups: Slow lfow or no relfow group, the patients with TIMI grade≤2,n=43 and Normal lfow group, n=173. Receiver operating characteristic (ROC) curve was performed to assess the best cut-off value for MHR predicting slow lfow or no relfow with its sensitivity and speciifcity; Logistic regression analysis was conducted to studied weather MHR could be used as an independent risk factor for coronary slow lfow or no relfow in STEMI patients after PCI. Results: Compared with Normal lfow group, Slow lfow or no relfow group had the higher MHR (18.6±9.8) vs (10.9±5.5), P<0.001. Univariate Regression analysis indicated that MHR was a risk factor of slow lfow or no relfow occurrence (OR=2.22, 95% CI 1.58-3.28); multivariate regression analysis presented that MHR was an independent risk factor of slow lfow or no relfow occurrence (OR=1.55, 95% CI 1.01-2.38). ROC curve showed that the best cut-off value for MHR predicting slow lfow or no relfow occurrence was 13.37 with the sensitivity and speciifcity at 67.4% and 70.5% respectively, the area under curve (AUC) was 0.734, 95% CI 0.646-0.822. Conclusion: MHR was an independent risk factor for slow lfow or no relfow occurrence in STEMI patients after PCI.

3.64 mmol/L)received 8-week double blind comparisons treatment.After treatment we randomly selected 15 patients for each group.The fasting serum lipid was measured at the end of 4-week and 8-week,and the serum hs-CRP and IL-6 were measured too.Meanwhile seven hypercholesterolemic patients from rosuvastatin group and seven ortholiposis out-patients were included.These patients were treated with statins for 4 weeks.The erythrocytes before and after treatment were kept to be measured the FA.Results (1)Compared with the pretreatment,the level of LDL-C,TC,hs-CRP remarkably reduced in two treatment groups at the end of 4-week therapy.These inflammatory parameters did not further reduced,but hs-CRP reduced further at the end of 8-week therapy.(2)The percent age of SFA was obviously higher in hypercholesterolemic group than ortholiposis group.On the other hand,PUFA and ?-3 FA were obviously lower.After 4-week treatment the percent age of SFA reduced,while PUFA and ?-3 FA increased significantly in hypercholesterolemic group.Conclusion (1)Statins reducing inflammatory factors is independent of lipid-modifying ability.(2)Rosuvastatin treatment can effectively reduce the level of LDL-C and TC,and no significant difference in the effect is observed between two treatment groups.(3)The erythrocyte membrane FA proportion was obviously different in hypercholesterolemic group from ortholiposis group.Statins treatment could make the proportion of FA in erythrocyte membrane restore normal.
